Immunization
requirements
Note:
Childhood immunization records will meet some
requirements.
If
you do not have immunization records, it is least expensive
to go ahead and take a MMR booster, Td, and Varicella
vaccine (or document having chickenpox) and start the
Hepatitis B series than it is to do lab testing to prove
serological immunity.
All
students born on or after January 1, 1957 must show proof of
either:
MEASLES
I) 2
doses of Measles vaccine administered since January 1, 1968,
and on or after their first birthday and at least 28 days
apart. (Measles is part of a
MMR vaccine).
II) at least one dose of measles vaccine administered on or after
their first birthday, which must be received by students
enrolled in health-related courses prior to direct patient
contact and completion of the measles requirement must be
accomplished as rapidly as is medically feasible; or
III)
serologic
confirmation of measles immunity or serologic evidence of
infection (Lab
testing showing immunity)
RUBELLA
Same
as measles requirement stating one dose after first birthday
or serologic evidence of immunity. (Rubella is part
of the MMR vaccine)
MUMPS
Same
as measles and rubella – one dose after first birthday or
serologic evidence of immunity. (Mumps
is part of the MMR vaccine)
HEPATITIS
B
Complete
series of Hepatitis B required
prior to start of direct patient care.
(Series takes a minimum of 6 months, so students need
to have two of the three vaccines done prior to entering the
clinical rotation. Start
first vaccine prior to first day of class).
POLIO
Polio
vaccine is not required.
TETANUS/DIPTHERIA
Tetanus/Diptheria
toxoid (Td) is required for all students and a dose must
have been received within the past ten years.
VARICELLA
(Chickenpox)
One
dose is required if student received the dose PRIOR to 13
years of age.
Two
doses are required if student was not vaccinated before
their 13th birthday.
All
doses must have been received AFTER the 1st
birthday.
OR
A
history of varicella illness (chickenpox) validated by the
student, the student’s parent or the student’s physician
or serologic confirmation of varicella immunity is
acceptable in lieu of vaccine. (Lab test showing immunity)
Documentation
with wording such as, “This is to verify that (name of
student) had varicella disease (chickenpox) on or about
(date) and does not need varicella vaccine.”
This statement must be signed with the appropriate
signature, as listed above.
(Complete
the Varicella statement form provided if you have had
the chickenpox disease and submit it with other required
records.)
A
recent TB skin test is required with results done less than
60 days prior to the beginning of the class.
